Fear of pain or further injury, as exemplified by fear-avoidance beliefs in the Fear-Avoidance Model, can lead to disability by promoting the avoidance of expected activities. Numerous studies have investigated the relationship between fear avoidance, pain, catastrophizing, and disability among those with chronic neck and back pain; however, research focusing on burn survivors remains scarce. To meet this demand, the Burn Survivor FA Questionnaire (BSFAQ) was formulated (1), but it lacks validation. Therefore, the primary focus of this study was to assess the construct validity of the BSFAQ questionnaire in a population of burn survivors. The research sought to understand the relationship between functional ability (FA) and (i) pain severity, (ii) catastrophizing behavior, and (iii) disability, assessing burn survivors at baseline, three months, and six months post-burn, focusing on the six-month assessment. To assess construct validity, a prospective mixed-methods strategy compared quantitative BSFAQ scores against qualitative interviews conducted with 31 burn survivors. These interviews delved into their lived experiences, the goal being to establish whether the BSFAQ effectively distinguished individuals holding fear of recurrence (FA) beliefs from those who did not. A retrospective chart review yielded data on pain intensity (Numeric Rating Scale), catastrophizing (Pain Catastrophizing Scale), and disability (Burn Specific Health Scale-brief) for burn survivors (n=51), pertaining to the secondary objective. Based on qualitative interview data, participants categorized as fear-avoidant demonstrated significantly different BSFAQ scores (p=0.0015) compared to participants categorized as non-fear-avoidant in the Wilcoxon Rank Sum Test. The accuracy of the BSFAQ in predicting fear avoidance reached 82.4% as per the ROC curve. Consistent with the FA model, burn survivors who exhibit fear avoidance are more prone to reporting higher pain levels early in their recovery. These elevated pain levels are intertwined with a persistent pattern of catastrophizing thoughts, culminating in greater self-reported disability. While the BSFAQ exhibits construct validity and accurately forecasts fear-avoidant behavior in burn survivors, further investigation into its clinimetric properties is warranted.
